Blue Earth, Minnesota, inside a church- We see a priest reading, when someone enters- it’s Meg. Meg tells the priest she needs to talk. She begins to confess her sins to him, including the fact that she slit someone’s throat and ripped his heart out! Her eyes go black and the priest says he knows what she is. The priest runs, but Meg follows. She asks about the Winchesters and the priest says he’ll never tell. Meg slits his throat.
Manning, Colorado- John, Dean and Sam are in the cabin discussing the demon. John shows his sons all he knows. Apparently, the demon leaves a trail. It goes after families. In particular, it goes after six month old babies. Sam feels guilty his mom and Jess died because of him. John says he’s seen signs of the demon coming and maybe for once they can stop a death. This time the signs are in Salvation, Iowa.
On the road- John’s pickup takes the lead with the Impala behind. Abruptly, John pulls over. He’s had a call from Caleb to say their friend Pastor Jim is dead. It looks like the demon may know they’re close. John says they need to hurry. They should split up and check hospitals for kids who will soon be sixth months old- that’s where the demon will strike next. He also says ‘This ends, now.’ Ominous words…They drive off.
Salvation Women and Childrens’ Hospital- John puts on a fake badge and enters, looking for records. Cut to Sam, checking on birth records elsewhere. Cut to Dean, chatting up a receptionist and also masquerading as a cop to check records.
Medical Center- Sam exits after looking up birth records. Just out the door he has a vision of a kid’s room, a woman with her baby, and then the demon. Sam tries to figure out where the house is he saw from the sound of a train. Eventually, he finds it.
A young woman, Monica – the one from his vision, is outside with her kid, Rosy. Sam goes over and chats with her. He finds out the kid is indeed six months old to the day… Sam tells Monica to take care of herself and walks away. Just then, he has another vision, this time clearer. Monica is killed by the demon in the same way his mom and Jess were!
Room- Sam tells John and Dean about his vision. John is shocked because he didn’t even know Sam had visions! At first he’s a little angry but concedes he isn’t around a lot to find things out. Sam cuts in and says the demon is coming tonight and then the phone rings. Sam answers. It’s Meg, and she wants to speak to John. Reluctantly, Sam puts his dad on.
Meg has Caleb and tells John if he doesn’t hand over the special Colt she’ll kill him. John says he doesn’t know about the Colt, and Meg slits Caleb’s throat. She then tells John if he doesn’t meet her and hand the gun over, she’ll kill more of the Winchesters’ friends. John gives in and agrees to meet her that night at a warehouse in Lincoln. After Meg hangs up, John explains to the boys that he’s going to hand over a fake gun. Meanwhile, it will be up to Sam and Dean to save the family and baby with the real gun. He just wants it to be over.
Outside- John is getting weapons from his trunk with Sam when Dean pulls up with a Colt bought from an antique store. Dean hands over the gun, telling his dad Meg has set up a trap. John knows this, but still needs to buy time. He tells the boys they only have four real bullets left for the special Colt, so they need to make every shot count. He tells them he can’t be there, but they should finish what he started. John hands the real Colt to Dean and they bid their farewells. John drives away in his pickup.
Warehouse in Lincoln- John climbs from his pickup and looks around. He spots a water tower- and inside the building, lots of pipe work.
Impala- Dean and Sam are watching Monica’s house. They discuss what to tell the woman if they have to go in the house. They also worry about their dad.
Warehouse in Lincoln- John climbs the water tower and opens the hatch. He pulls out a rosary and blesses the water.
Impala- Sam says it doesn’t seem real that they’re finally facing the demon. He tells Dean thank you for everything, in case anything happens to him. Dean says nothing is going to happen to anyone- except the demon!
Warehouse in Lincoln- John meets Meg. It seems she’s brought backup. A guy walks from the shadows. John hands over the gun and Meg and her buddy try to figure out if it’s real. The guy shoots Meg, knowing if it’s the real Colt it will kill her. When it doesn’t, they know John has handed over a fake! John pretends he didn’t know the gun wasn’t real and makes a run for it! As he dashes down a corridor, he turns opens valve on a pipe, releasing the holy water he blessed! Meg and her buddy can’t pass through it.
Impala- Dean tries calling his dad, but there’s no answer. Sam notices the radio has gone weird, and outside the car a wind has whipped up. Lights on the street are going on and off…the demon is here! The boys make a dash for the house.
Warehouse in Lincoln- John escapes the warehouse to find his pickups tires have been slashed. There’s nowhere to run but down an alleyway.
Monica’s House- Dean and Sam enter the house but Monica’s husband, Charlie attacks them thinking they’re burglars. Dean takes care of hubby, while Sam goes to save Monica and Rosy in the nursery. The demon pins Monica to the roof, but Sam bursts in just in time. He shoots at the demon with the Colt, but it vanishes before the bullet hits. Sam quickly grabs Monica and drags her out while Dean grabs Rosy just before her cot bursts into flames.
Alleyway- John tries to make a cell call but is pinned to the wall by Meg’s evil buddy. He’s caught!
Monica’s House- Dean and Sam get everyone clear just before the upper story bedroom explodes in a fireball. Sam looks up and sees the demon within the flames. He wants to go inside and fight it, and Dean has to pull him back!
Room- Dean calls their dad but there is still no reply. Sam is still moping because Dean stopped him going back in the house to fight the demon. The brothers have an argument because Dean says he won’t let Sam sacrifice his life to kill the demon. They make up and Dean tries to call their dad again. This time, Meg answers. She tells dean ‘You’re never going to see your father again…’
